What is the criteria for referral to the Peace Hospice?
The criteria for referral to the Peace Hospice Day Care, Inpatient Unit, Hospice at Home and Outpatient Complementary therapies is as follows:-
- The Hospice accepts referrals for all patients resident within SW Hertfordshire.
- All patients must be over age 18 with life-threatening disease whether cancer or non-cancer.
- Patents will be accepted with symptom control needs, psychological or social needs or when those providing day-to-day care assess a need for specialist palliative care
- Patient agrees to referral
- Referrals of patients will be accepted from general practitioners, district nurses, hospital and community Macmillan nurses and hospital consultants.
- Referrals faxed to the Hospice and telephone enquiries will be responded to by the Referral Co-ordinator on the day they are received and will be replied to within 2 working days. The GP will be informed in writing of any admissions made.
Admission Criteria
- All admissions will be subject to the above criteria, based upon need and informed by the potential benefit to the patient and their family and subject to bed availability.
- Patients will be admitted for a short period of ongoing assessment and the patient’s condition will clearly determine the length of stay and future care needs. Admissions are expected to be on average 1-2 weeks duration. The Peace Hospice does not offer long term care.
- The information you provide within this referral form is very important to us and is often the only way we are able to make a decision with regard to your request for a hospice service. Clear, concise and relevant information assists us in processing your request and we also ask you to fax all relevant recent medical letters, and investigation results over to us to help in our decision. Must be faxed as well for data protection cannot be received electronically.

Breathlessness Clinic Referral
- The Breathlessness clinic is run by a physiotherapist and a specialist nurse and is aimed at patients whose quality of life could be improved by undergoing a programme of care which includes exercises and coping strategies for breathlessness.
- Patient referrals will be accepted from general practitioners, district nurses, hospital and community Macmillan nurses and hospital consultants.
